- Applications must be filed, and all fees must be paid, by the payment due date to avoid late charges.
- The last day to submit a (Qualified Tuition
Reduction) form for processing is the last
day of classes of the semester or
session for which the tuition reduction
will be applied. Late charges will be
applied after the
payment
due date, and will not be
waived.

To process a QTR form:
- Obtain the QTR
application form from the above links, or from your department's payroll
representative.
- A new QTR form must be completed and approved
each semester.
- You must be registered for classes, and all
prior semester charges must be paid.
- Complete the form, obtain the required
signatures, and mail or hand carry the form to
either:
